View Behavioral & Mental Health ServicesThe Transition Center - Pathway to Positive Futures is a one-stop shop for our youth and their families to access resources and receive assistance with transitioning a youth’s release from Juvenile Hall back into their communities. Additionally, the Positive Futures Transition Center staff members help connect all youth and their families (justice involved, At Promise, and non-justice involved youth) to resources including re-entry plans, education and school enrollment, mental health care/resources, life coaching and case management, and other referrals to community-based organizations.
These resources are available to our community at no cost. If you or your child would like a referral to any of these services, please contact the Transition Center at 510-667-4304, or email probationtransitioncenter@acgov.org.
