Lincoln - Project Permanence Wraparound Program:
- Serving: All Alameda County youth on probation who have serious mental health or behavioral issues.
- Description: Family support, collateral supports, individual support to the referred youth, and linkages to resources. Interventions integral to the model are 1:1 supports directly to the youth, family support, monthly family team meetings, and intensive case management services. Seneca Family of Agencies - Intensive Case Management (ICM):
- Service Availability: Only for youth participating in Collaborative Court.
- Description: Treatment approach similar to a wraparound model. Multiple team members, including a clinician, parent partner, and youth counselor, work with a youth for a period of 12 to 18 months or until dismissal from probation. Youth and their caregivers meet with team members multiple times per week. Centerforce Youth Court (DPN):
- Description: As part of Centerforce’s Youth Court programming for first-time juvenile offenders, youth who would otherwise be referred for prosecution to the District Attorney are given the option and opportunity to have case management with their parent(s), their case handled by peers in a court-like setting, and adjudicated (including a penalty imposed, e.g., community service and a CYC-based class such as Anger Management).
